Продолжительность оказания паллиативной помощи детям определяется характером нозологической формы и нередко растягивается на годы. Поэтому своевременная диагностика, профилактика и терапия стоматологической патологии приобретают первостепенную значимость, поскольку выраженность основного заболевания значительно затрудняет стоматологические вмешательства. Исследование состава бактерий полости рта у детей с ограниченными возможностями здоровья, как одного из дополнительных неинвазивных методов ранней диагностики факторов риска развития кариеса зубов, несмотря на достигнутые успехи в этой области, требует согласованного понимания механизмов, определяющих процессы структурообразования зубных отложений и патогенеза заболеваний твердых тканей зубов у данной группы граждан. Целесообразность использования зубного налета в качестве объекта исследования объясняется его сложной физиологической ролью в развитии заболеваний твердых тканей зуба, а также доступностью получения диагностического материала.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Для анализа микробиоты зубного налета проведено комплексное клиническое стоматологическое обследование 15 детей в возрасте 6 лет. Микробиологический статус заключался в определении общего микробного числа и идентификации штаммов бактерий и грибов с определением ряда основных характеристик. В плоскодонных планшетах формировали биопленки выделенных штаммов, после чего обрабатывали их настоями лекарственных трав, растворами фурацилина и хлоргексидина. Толщину биопленок изучали путем их окрашивания кристаллическим фиолетовым с последующей его экстракцией спиртом.</p></sec><sec><title>Результаты</title><p>Результаты. Бактериологическое исследование зубного налета 15 проб позволило выделить и идентифицировать 45 штаммов бактерий и грибов. В структуре микробного сообщества доминировали представители рода Staphylococcus – 16 штаммов (35,6%), изолированные из 13 образцов (86,7%). Подавляющее большинство выделенных штаммов обладает высокой способностью к образованию биопленок. Традиционные средства (хлоргексидин и фурацилин) оказывали слабое влияние на зрелые биопленки или не влияли на них. Настои всех изученных лекарственных трав эффективно снижали толщину биопленки коагулазаположительных стафилококков, но не оказывали влияния на этот параметр у коагулазоотрицательных видов. Установлено, что настои ромашки и крапивы обладают антибиопленочным эффектом в отношении нейссерий, настои календулы и крапивы – против энтеробактерий, а календулы и ромашки – против биопленок Candida.</p></sec><sec><title>Заключение</title><p>Заключение. При бактериологическом исследовании зубного налета детей дошкольного возраста, получающих паллиативную помощь, выделены бактерии разных видов, с преобладанием энтеробактерий и стафилококков. Традиционные препараты (хлоргексидин и фурацилин) оказывают минимальное воздействие на толщину биопленки клинических штаммов. Использованные в исследовании настои лекарственных трав проявляют выраженные антибиопленочный эффект.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. The duration of palliative care in children is determined by the underlying medical condition and often extends over many years. Therefore, timely diagnosis, prevention, and treatment of dental diseases are of critical importance, as the severity of the primary condition can significantly complicate dental interventions. Despite notable advances in this field, investigation of the oral microbiota composition in children with disabilities as an additional noninvasive approach for the early identification of caries risk factors still requires a unified understanding of the mechanisms underlying dental plaque formation and the pathogenesis of dental hard tissue diseases in this patient population. The rationale for using dental plaque as the study material is based on its complex physiological role in the development of dental hard tissue diseases, as well as the accessibility of diagnostic sample collection.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. To analyze the dental plaque microbiota, a comprehensive clinical oral examination was performed in 15 children aged 6 years. Microbiological assessment included determination of the total microbial count and identification of bacterial and fungal strains, along with evaluation of their basic microbiological characteristics. Biofilms of the isolated strains were formed in flat-bottom microtiter plates and subsequently treated with herbal infusions, furacilin solution, and chlorhexidine. Biofilm thickness was assessed using crystal violet staining followed by ethanol extraction.</p></sec><sec><title>Results</title><p>Results. Bacteriological examination of 15 dental plaque samples enabled the isolation and identification of 45 bacterial and fungal strains. The microbial community was dominated by representatives of the genus Staphylococcus — 16 strains (35.6%) isolated from 13 samples (86.7%). The majority of the isolated strains demonstrated a high capacity for biofilm formation. Conventional antiseptic agents (chlorhexidine and furacilin) showed little or no effect on mature biofilms. Infusions of all tested medicinal herbs effectively reduced the biofilm thickness of coagulase-positive staphylococci but had no significant effect on coagulase-negative species. Chamomile and nettle infusions demonstrated antibiofilm activity against Neisseria spp.; calendula and nettle infusions were effective against enterobacteria; and calendula and chamomile infusions reduced Candida biofilms.</p></sec><sec><title>Conclusion</title><p>Conclusion. Bacteriological analysis of dental plaque in children receiving palliative care revealed a diverse microbial composition, with predominance of enterobacteria and staphylococci. Conventional antiseptic agents (chlorhexidine and furacilin) showed minimal effects on the biofilm thickness of clinical strains.
