Современный взгляд на пародонтит как на дисбиотическое заболевание, возникающее в результате изменения микробного состава поддесневой области, рассмотрен в систематическом обзоре.</p></sec><sec><title>Цель</title><p>Цель. Изучить новую парадигму развития генерализованного пародонтита.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Для изучения отбирались рандомизированные контролируемые исследования (РКИ), в том числе кластерные РКИ, контролируемые (нерандомизированные) микробиологические и клинические исследования микробиома полости рта у взрослых пациентов с генерализованным пародлонтитом за последние 10 лет.</p></sec><sec><title>Результаты</title><p>Результаты. Переход от симбиотической микрофлоры к дисбиотическому патогенному сообществу запускает воспалительную реакцию хозяина, которая способствует развитию заболеваний пародонта. Современные представления о пародонтопатогенных бактериях диктуют новые требования к лечению заболеваний пародонта. Во второй части обзора рассмотрены микробные профили пародонта при различных нозологических формах заболевания, механизмы иммуного ответа и подходы к лечению заболевании пародонта с позиции биопленочной инфекции.</p></sec><sec><title>Выводы</title><p>Выводы. Как следует из современной литературы, пародонтит в определенной степени вызван переходом от гармоничного симбиотического бактериального сообщества к дисбиотическому. Последние научные исследования показали, что один микроорганизм не способен вызывать заболевание, что к развитию патологии приводит деятельность микробного сообщества в целом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. The modern view of periodontitis as a dysbiotic disease that occurs as a result of changes in the microbial composition of the subgingival region is considered in a systematic review.</p></sec><sec><title>Purpose</title><p>Purpose. To study a new paradigm of development of generalized periodontitis.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Randomized controlled trials (RCTS) were selected for the study, including cluster RCTS, controlled (non-randomized) microbiological and clinical studies of the oral microbiome in adult patients with generalized periodontitis over the past 10 years.</p></sec><sec><title>Results</title><p>Results. The transition from a symbiotic microflora to a dysbiotic pathogenic community triggers the host's inflammatory response, which contributes to the development of periodontal diseases. Modern ideas about periodontal pathogenic bacteria dictate new requirements for the treatment of periodontal diseases. The second part of the review examines the microbial profiles of periodontal disease in various nosological forms, the mechanisms of the immune response and approaches to the treatment of periodontal disease from the perspective of biofilm infection.</p></sec><sec><title>Conclusions</title><p>Conclusions. As follows from modern literature periodontitis is to a certain extent caused by the transition from a harmonious symbiotic bacterial community to a dysbiotic one.
